105年起醫院大力推動並鼓勵召開跨領域專業團隊溝通會，106-107年共召開3,164場次跨團隊會議，介入含括營養、復健、藥劑師等多職類。106-107年出院後3日內再急診率由3.21%降至3.09%，非計畫性住院案件出院後14日內再住院率由6.25%降至4.52%，有效提升個案及家屬返家後照護品質。
